1. SCAN the QR Code to access the school’s registration form

2. You are required to have the following documents on hand:
1) ONE (1) passport size photo
2) Birth certificate (An affidavit is required if there is no name on the birth certificate)
3) Immunization Card
4) Proof of citizenship (if born outside of Trinidad and Tobago)
5) Valid student permit (if a non-national)
6) Legal Parent or Guardian's National ID card.
Documents 1) through 6) are required to be scanned and uploaded at the end of the registration form. Scanned documents or a clear picture format will be accepted.
3. Read and answer ALL questions carefully. (You MUST enter a response for any questions with the red asterisk (star) *)
4. Once you have entered and VERIFIED all the required information, please click the SUBMIT button.
POINTS TO NOTE:
· DO NOT SUBMIT more than ONE (1) RESPONSE per student.
*If you have made an error on the form, make a note of it and inform the teacher on Registration day.
*If you have more than one child registering, you can proceed to submit another response.
· Parents requesting transfers or not accepting their position MUST visit the Administration Office of St. Joseph Secondary School on Thursday 4th July 2024 during the hours of 9:00 a.m. to 1:00 p.m.
You will be required to attend REGISTRATION DAY carded for:
Thursday 4th July, 2024 at 9:00 a.m.
You will then be required to visit the Administration Office of St. Joseph Secondary School on Thursday 4th July 2024 at 9:00 a.m. with your original documents for verification. A contribution of $600.00 will also be collected to complete the registration process.
If you are unable to upload the documents listed above, PLEASE walk with the original and a copy of the documents on Thursday 4th July 2024 at 9:00 a.m. A $600.00 contribution will be collected to complete the registration process.
Student (s) must be accompanied by a LEGAL parent or Guardian. If the LEGAL parent or Guardian is unable to be present for registration day then the representative MUST provide an authorization letter along with a copy of the LEGAL parent/Guardian’s Identification.
The following contributions are required:
School Identification Card - $100
PTA fee - $100
Stationery and Printing Services - $200
School Internet Connectivity and Online Resources - $200
P.E. Uniform t-shirts will be on sale at a cost of $65.00. Black Track Pants can be bought at any Sports Stores or Bookstores nationwide.
NOTE: The cost of the P.E. Uniforms is not included in the Contribution.
